How to Get Started on Formed

• Go to www.formed.org
• Click the blue button on the homepage, “Formed for individuals”
• Enter the email address you would like to use to create your account.
• Click “continue to sign up”
• Follow the steps to continue signing up.
• When prompted to enter your parish, type “Our Lady Queen of Martyrs”
